Admission Process

SRVS is licensed and funded by the State of Tennessee, Department of Human Services (Division of Rehabilitation Services), and the Department of Intellectual and Developmental Disabilities, and must follow all requirements for admission as set forth by current state regulations. SRVS understands the urgency and need for services of all applicants. However, there is no “quick and easy” way to provide services immediately upon request.

The office of Admission and Discharge will make every effort to process and obtain approval for all applicants in as timely a manner as possible. It is important to remember that the admission process takes time. SRVS appreciates your patience and cooperation throughout the journey.

Eligibility

Services are available to residents of West Tennessee with disabilities. Certain conditions set by the Tennessee Division of Intellectual Disabilities Services must be met before admission to any SRVS residential or day program. If admission is to be at a future date due to lack of available slots, the applicant will be placed on a waiting list. Admission is offered as vacancies occur.

Determining Eligibility

The following assessments/documents must be provided to determine eligibility:
  • SRVS application
  • Psychological examination
  • Certification of eligibility with Division of Rehabilitation Services (DRS) for the Community Employment Services program only
Once eligibility has been established and funding approved by the State, the following assessments/documents must be provided prior to admission:
  • Current physical examination
  • Current dental examination
  • Current optical examination
  • Other records as deemed necessary

Admission to any SRVS program is granted initially on a 90-day trial basis. The trial period is used to evaluate the appropriateness of the placement, the individual’s interest and time for all parties to adjust. If, at any time, adjustments are needed in programming, SRVS will work closely with the State, the individual’s family and the circle of support to connect the individual to the most appropriate services and supports available.
