Elderly & Adult Disability Services

The Elderly and Adult Disability Services program is an approved provider of CHOICES. We are a network of home and community based services designed to assist the elderly and adults with disabilities, enabling them to live independently for as long as possible. Our program is built on a solid reputation of providing quality service to each and every individual.

Who We Support

  • Adults aged 65 and older
  • Adults 21-64 who are living with a physical disability
  • Adults who are having a difficult time carrying out activities of daily living
  • Any resident of Tennessee
  • Support Services
Home and Community based services are available to individuals who desire both in-home and supportive assistance as an alternative to nursing home placement.

Adult Learning Center

Provides supervised care and activities during the day such as life skills, training, sensory room, arts and crafts, music, recreation, therapy, media room and community outings.

Homemaker Services

Provides assistance in the home with routine household chores.
  • Sweeping, mopping, dusting
  • making the bed and washing dishes
  • personal laundry and ironing
  • meal preparation
  • grocery shopping
  • errands such as getting prescriptions filled

Personal Care Services

Provides assistance in the home with bathing, dressing, feeding and other activities of daily living including some assistance with household cleaning and the maintenance of a safe home.

Respite Care

Provided on a short-term basis when there is an absence of, or need for, relief for the caregiver. For example, respite might be provided when the caregiver is ill or hospitalized or when the caregiver takes vacation. Respite is limited to a maximum of (9) days per waiver year.
